LIMITED POWER OF ATTORNEY The undersigned, Robert Chance DeWitt, MD (the “Reporting Person”), hereby authorizes and designates each of R. Blake Chatelain; Isabel V. Carriere; M. Allison Johnson; Julia E. Callis; Clinton H. Smith; Kelly C. Simoneaux; and Jenese C.
